A direct request or “referral“ for service is made to our agency, through the client, physician, caregiver, health facility representative, hospitals, skilled nursing facilities, case managers, social workers, family members, friends, or community member.
Someone to Love, LLC brings experienced and knowledgeable services right to your door; beginning with a licensed registered nurse who will contact you to set up an in-home consultation to ensure you receive the most personalized care that you or a loved one needs. If you decide you want to move forward with our care team, a more personalized health assessment will be conducted to identify areas where care is needed the most.
Personalized Care Plans
Your plan of treatment is a personalized care plan that you help design along with trained and licensed professionals. The care plan will address your health concerns and wellbeing. Our goal is to help you follow your treatment plan as ordered by your physician. Each individual will require their own unique plan of care which will be modified as you advance. The care plan covers pertinent diagnoses, prognosis, nutritional requirements, medication review, and treatments to help you understand and monitor prescriptions you are taking. A registered nurse and or/physical therapist offers services ranging from frequent help with equipment, to rehabilitation potential, and functional limitations with transfers, while working to maintain top safety measures that protect you from injuries.
Skilled Care Specialists/Caregivers
Clients are assigned to appropriate staff members by the registered nurse or under the supervision of the nurse according to location, clinical needs of the client, and the qualifications and availability of staff. All applicants go through a face to face interview with our highly trained management team, FBI/criminal background checks, Penn-dot driving records check, drug screenings, and infectious disease screenings. Candidates are required to complete a comprehensive orientation program and regularly scheduled In-services and performance reviews.
Evaluation of the Care
The client's health needs/problems, plans and goals are reviewed by a coordinating nurse on an ongoing basis and revision of the plan of care is carried out as frequently as necessary to reflect the changing care needs of the consumer. All care providers/specialist are supervised by a registered nurse/clinical care manager. Regular visits by the nurse coordinator are made to clients home to ensure the highest level of care is being provided around the clock.
